FREE LESSON & ASSESSMENT FORM

Students taking lessons more than once per week receive a 5% discount on the second day and a 10% discount on the third day with a 3 month minimum enrollment. Siblings also receive discounts, at 5%, 10%, 15%, and 15% for 2nd, 3rd, 4th & 5th child, with minimum. Group students must move to another group when level is mastered.

We offer a satisfaction guarantee on all of our lessons, but there are no refunds for any reason. If you are not happy with any lesson you attend for the full 25 minutes, let us know the day of your lesson and we will give you another lesson for FREE. But we need your commitment to the lessons and to our program and your communication to us when you are not satisfied; therefore, there are no refunds in the hope that you will continue to work with us to ensure your satisfaction and delight!

I have given my email addresses, above, so the Swim Club can notify me of changes in teacher schedules, Open Swim changes, pool closure dates, substitutions, Safety Week and Love ‘Em reminders, changes in policies, expired accounts, etc. Without an email addresses, I may not be notified of these important events. (Please add us to your address book so they are accepted.)

Policies:

1. I have read and understand the shower policy, and we agree to take soap showers before entering the pool.
2. I realize that it is always recommended to get a physician´s approval before beginning any exercise regimen.
3. I understand there are certain risks involved with swimming and related activities, and I agree to assume all liability for my children and myself with regard to fault while at Academy Swim Club. I further agree to hold harmless the officers of Academy Swim Club, or any of its employees for any complications or injuries that may result from my children or myself attending Academy Swim Club. I also understand that although my child may pass certain skill levels during classes, children need supervision around or near water at all times and it is dangerous to assume that any child is "water safe".
4. I understand that although the Swim Club does all it can to keep my instructor on the same days and times, instructors may change without notice.  Parental request for changes should be limited, and may incur a $25 fee for more than 1 change every 3 months.
5. I understand that the Academy Swim Club must maintain a child-friendly atmosphere at all times and should I conduct myself otherwise, (i.e. cursing or screaming) I will be asked to leave and may not be allowed to return for a minimum of one year. There are no refunds, but my credit will be saved for me.
